Addison's Disease

The adrenal gland in the canine body is responsible for making several hormones, two of which are glucocorticoids and mineralocorticoids.
Glucocorticoids (better known as hydrocortisone, prednisone, etc) act as antiinflammatories and as catabolic agents, that is they promote the break down of protein, fat, etc for energy. They also have a variety of less well described functions. Mineralocorticioids are responsible for maintaining proper level of Potassium and Sodium primarily but other electrolytes as well. Addison's Disease is also known as adrenal insufficiency or hypoadrenalcorticism and is characterized by a lack or insufficient production of both glucocorticoids and mineralocorticoids.

School of Veterinary Medicine. University of California, Davis, is undertaking a study on Addison's disease in the Toller. This is Dr. Hughes' resident research project and she will be conducting it under the guidance of Drs. Bannasch (Genetics), Oberbauer (Genetics), Famula (Statistics), and Nelson (Internal Medicine)." 1. What are the symptoms of Addison's Disease in dogs? How is it diagnosed? How important is early detection? Addison's disease can present suddenly in young healthy pets, or in older pets that have been through major stress situations eg intensive care for road a traffic accident. Affected dogs are in severe shock with pale gums, slow heart rates, weak pulse, cool extremities and often have upset stomachs and weakness.
The clinician would start to get alarm bells ringing when considering the age, history and findings at examination. Bloods for electrolytes and biochemistries often show high potassium levels and elevation of the kidney enzymes. If the potassium is elevated, an EKG (ECG) would show a slow heart rate and depressed P waves (the part of the ECG showing the smaller heart chambers, atria, contracting)
Addison's disease is a life threatening condition that must be treated ASAP. Untreated dogs are usually dead within 24 hours from heart failure brought on by the increased potassium levels.

This is a relatively common complaint in cats. If you have 100 cats and feed them the same food, a few will develop crystals in their urine which look very similar to salt granules. Addison’s Disease
In Western world, 70% of all cases are due to autoimmune adrenalitis causing atrophic adrenal cortex . Main autoantibody is to adrenal enzyme, 21-hydroxylase. Half of patients have associated autoimmune disease. Two types of ‘polyglandular’ autoimmune syndromes: Type I – inherited autosomal recessive condition with defect in immunity – Addison's disease combined with chronic mucocutaneous candidiasis together with hypoparathyroidism (rare condition presenting in childhood). Type II – more common comprises Addison’s disease together with autoimmune thyroid disorder (usually hypothyroidism), hypogonadism and type 1 diabetes mellitus . Other causes of primary Addison’s disease are very rare including metastatic tumours, lyphoma and amyloid. Secondary hypoadrenalism Due to ACTH deficiency. Common clinical problem due to sudden stopping of exogenous steroids or failing to give steroid cover for intercurrent illness in those on long-term steroid therapy.

Blood results from pets with this condition often show dehydration, an increase in kidney waste products (azotemia, uremia), metabolic acidosis (too acidic a body) elevated blood potassium (hyperkalemia in 95% of cases), low blood sodium (hyponatremia) and occasionally elevated blood calcium. Urine specific gravity is low in seventy-five percent of the cases.
Some dogs also need to receive bicarbonate of soda by injection or orally to nutrilize acid. Addison's disease is treatable but not curable. Some breeds develop this disease more commonly than others. We call this predisposition. These breeds include Portuguese Water Spaniels, Great Danes, West Highland White Terriers, Rottweilers, Standard Poodles, and Wheaten Terriers.
