The Aviation Safety Division was created within the Transportation Safety Institute in 1971 to support the national program efforts and initiatives of the Federal Aviation Administration (FAA). In 1963 the FAA initiated Aircraft Accident Investigation training for FAA Aviation Safety Inspectors at the FAA's Mike Monroney Aeronautical Center. In 1971 this training was transferred to TSI's Aviation Safety Division where the training is still being performed. Thus, "The National Aircraft Accident Investigation School;" the premier aircraft accident investigation school, has been taught since 1963 with TSI proudly continuing the efforts and initiatives of the FAA. The Aircraft Accident Investigation and Aviation Safety training programs promote aviation safety by training accident investigators and aviation safety professionals. Introduction
It would be pleasant to say there have been no accidents. Unfortunately, as with many new types of aircraft, the Airbus A320/A330/A340 series has had its share. There have been fatal accidents with A320 aircraft in Bangalore, India; in Habsheim, in Alsace in France; near Strasbourg, also in Alsace in France; and in Warsaw, Poland. An A330 crashed on a test flight in Toulouse, killing Airbus's chief test pilot as well as the other crew on board. An A340 suffered serious problems with its flight management computer system en route from Tokyo to Heathrow, and further significant problems on approach to Heathrow. In late 1995 and early 1996, the B757 (not a fly-by-wire aircraft) suffered its first two fatal accidents in a decade and a half of service. Even transport aircraft which are not fly-by-wire have nowadays their share of safety-critical computer-based systems. New-generation flight management and guidance systems (the so-called `glass cockpit') and Full-Authority Digital Engine Controls (FADEC) are found on most newly-built aircraft.

This evaluation was initiated in response to a request from the Deputy Under Secretary of Defense (Installations and Environment), to conduct a process review of the Military Service's implementation of the DoD policy as stated in DoD Instruction 6055.7, "Mishap Investigation, Reporting, and Recordkeeping," April 10, 1989. The Deputy Under Secretary of Defense (Installations and Environment) asserted that improving aviation safety was a major objective of DoD. Military Department aviation safety programs focused on the reduction and prevention of aviation mishaps; however, aviation mishaps continued to injure people and caused property damage and losses. During FY 2000, the Services experienced a total of 57 class "A" aircraft mishaps, which resulted in 58 deaths and 44 aircraft destroyed. A class "A" mishap occurred when the reportable damage was $1 million or more; there was total destruction of the aircraft; or an injury resulted in a fatality or permanent total disability. Our overall objective was to determine whether the Services had implemented DoD policy for aircraft mishap investigations and reporting. Specifically, we evaluated:

Release of the Rand Accident Report
Thursday, 24 May 2001 The Commissioner for Civil Aviation, Mr. Trevor Abrahams has released the final accident investigation report of the fatal aircraft accident near Rand Airport on 6 December 1999, in which 10 lives were lost. The aircraft had taken off from Rand Airport on a commercial charter flight to Oranjemund in Namibia, when shortly after taking off, the pilot reported an emergency to the control tower and requested clearance to return to the airport to land. The aircraft crashed about 2 minutes after take off and everybody people on board lost their lives. The investigation into the accident revealed that the probable cause of the accident was the failure of a critical exhaust pipe segment, which in turn caused the right-hand engine to lose power/fail. It is was also found that the aircraft exceeded it's designed load limit. This overloaded condition of the aircraft was thus a significant contributory factor. According to Mr. Abrahams "this was a landmark accident investigation for the South African Civil Aviation Authority as it represents a whole new approach to investigating accidents through a detailed investigation not only of the immediate circumstances and conditions as in surrounding the accident, but also a close look at the chain of actions and inactions by all connected parties that could be identified however insignificantly, as having contributed to the immediate circumstances leading up to the tragic event. The accident investigation report details the factual information surrounding the tragic event including the history of the flight, the injuries to the persons on board, the damage to the aircraft, the pilot, the aircraft, the engines, meteorological information as well as the weight and balance of the aircraft for that flight and the communication between the pilot and the control tower at Rand Airport, amongst others.

The chain of events resulting in a serious incident begun when R1 and R2 prepared themselves for their work and noted that the westbound traffic from Russia flying over Finland flew at FL 380, whereas the southbound traffic from Northern Finland was at lower flight levels. They noted that there should be no separation problems between the two traffic flows. This initial assumption probably contributed to the fact that neither controller eventually noticed the two aircraft approaching each other at FL 360 which deviated from their original flight levels. Another contributing factor was that the Short Term Conflict Alert (STCA) system was not in use in the air traffic control at the moment. The investigation commission made three safety recommendations of which two were to the Finnish Civil Aviation Administration (FCAA). The FCAA should take the existing STCA systems into use to the extent they can be used without a great number of nuisance alerts. The FCAA should also to acquire a well-functioning STCA system to all ATC units which give radar services. Furthermore, the investigation commission recommended that Finnair Oyj should take into account in the pilots’ TCAS training the risks associated with changing from level flight to climb or descent in accordance with ATC clearance during TCAS TAs. The draft report was sent for comments to the UK AAIB, the Finnish flight safety authority, the ANS department of the Finnish CAA, the ANS centre for South Finland and Finnair. The Finnish flight safety authority and Finnair had no comments. The comments of the ANS centre for South Finland and the UK Accredited Representative have been taken into account in the report. Tampere ACC activated the existing STCA system soon after the incident for use above FL 200. The ANS department of the FCAA noted that a better STCA system is estimated to be taken in use with a new Eurocat 2000 v2 system in November 2003 in South Finland and in summer 2004 in North Finland. The comments of the ANS department are enclosed in the Finnish report and available in Finnish at AIB Finland.
