It is important to realize that laboratory results may be outside of the so-called "normal range" for many reasons. These variations may be due to such things as race, dietetic preference, age, sex, menstrual cycle, degree of physical activity, problems with collection and/or handling of the specimen, non-prescription drugs (aspirin, cold medications, vitamins, etc.), prescription drugs, alcohol intake and a number of non-illness-related factors, Any unusual or abnormal results should be discussed with your physician. It is not possible to diagnose or treat any disease or problem with this blood test alone. It can, however, help you to learn more about your body and detect potential problems in early stages when treatment or changes in personal habits can be most effective. Our lab, like almost all labs, sets the normal result range for a particular test so that 95% of our healthy patients fall within the normal range. That means that 5% of our healthy patients fall outside of the normal range, even when there is nothing wrong with them. Thus an abnormal test does not necessarily mean that there is something wrong with you. Statistically if you have 20 or 30 individual tests run as part of a panel, chances are 1 or 2 will be slightly outside the normal range. Part of what you see your doctor for is to interpret whether or not these changes are significant.

Sources at Johns Hopkins Medical Center in Baltimore revealed late Thursday morning that the lump removed from Mrs. Gore's thyroid gland in a brief surgical procedure is not cancerous. Mrs. Gore had minor surgery at the medical center Tuesday to remove the lump. Her attending doctor, endocrinologist Robert Udelsman, was scheduled to hold a press conference later Thursday to discuss the results of a biopsy on the nodule. All indications were that the lump is benign, meaning Gore will likely not suffer any long-term ill effects of the growth.

Moreover, the likelihood of developing a thyroid nodule increases with age and in part represents the aging process of the thyroid gland. Although most nodules are harmless and do not require any treatment at all, approximately 5% of all nodules are cancerous and therefore require attention. Thyroid nodules occur more frequently in women than in men, however the incidence of can cerous nodules is greatest in men. Despite the low incidence of cancer, all nodules should be evaluated by a physician to determine their status. Although the cause of most thyroid nodules is unknown, some risk factors for developing nodules include a lack of iodine in the diet, which can cause thyroid enlargement, family history of benign thyroid nodules, and pre-existing thyroid disease (e.g. Hashimoto's thyroiditis).

The thyroid test, T4 that is routinely run on birds is a RIA test, which is primarily used for canine and feline testing. Unfortunately, since the normal thyroid levels for avian species are so much lower than those found in dogs and cats; the test is not going to be the most accurate for birds. An experimental test has been developed that is not commercially available yet for avian thyroid testing. Until that methodology becomes available, we must make do with the testing at hand. The RIA test that is currently being used for thyroid testing is accurate down to approximately 0.5 ug/dl. At this point it begins to lose its linearity and accuracy, down to 0.2 ug/dl. Once the value drops below 0.2 ug/dl, the value becomes unreliable. Adding to the diagnostic dilemma is the fact that the normal range of T4 varies considerably between the different species of birds, even within birds of the same Genus. Hypothyroidism should never be diagnosed based on ONE low T4 value. Very few documented cases of hypothyroidism have been diagnosed in birds. To accurately diagnose hypothyroidism, a TStim should be performed. However, the necessary hormone, THS, is a human product that costs approximately $1,000 for a box containing two vials (Genzyme). A bovine derived THS is available, but it is NOT produced for use in animals, and is made for experimental use only. With concerns about BSE, I would not recommend using this product in avian patients.

These conditions are called hyperthyroidism (e.g., Grave's disease) and hypothyroidism. Sometimes the thyroid gland can become enlarged from over-activity (as in Grave's disease) or from under-activity (as in hypothyroidism). An enlarged thyroid gland is often called a "goiter." Sometimes an inflammation of the thyroid gland (Hashimoto's disease) will cause enlargement of the gland. Patients may develop "lumps" or "masses" in their thyroid glands. They may appear gradually or very rapidly. Patients who had radiation therapy to the head or neck as children for acne, adenoids, or other reasons are more prone to develop thyroid malignancy. A doctor should evaluate all thyroid "lumps" (nodules).
