While the United States Surgeon General at first replied that the Pennsylvania Hospital's offer of beds would not be needed, a later report by hospital superintendent Daniel Test recorded otherwise: During the past month (July, 1898) the facilities of the Hospital have been tried as never before since its capacity has increased, and we have again had an opportunity to witness the ability of the Hospital to meet emergencies. On the 5th inst. ["inst." was a 19th century abbreviation for "the present month"], 87 Soldiers were admitted and although they arrived 18 hours before expected, every one was placed in bed in one hour. On the 15th inst., we received 43 more and on the 20th inst., 49 arrived .... In September and October, the record showed an additional 113 soldiers were admitted. Altogether, 292 soldiers were cared for, some of them transported from ports of entry other than Philadelphia on special Red Cross trains manned by Pennsylvania Hospital staff. The large majority of them were ill with typhoid fever.

Many people agreed with historian Frederick Jackson Turner when he predicted that "American energy will continually demand a wider field for its exercise." Turner believed that for the American spirit to thrive, it needed a frontier. Influential leaders like Senator Henry Cabot Lodge from Massachusetts, Assistant Secretary of the Navy Theodore Roosevelt, and newspaper magnate William Randolph Hearst took Turner's ideas one step further. They believed that America was in fact destined to expand beyond its borders. Expansion, as Roosevelt later explained, meant "not only the extension of liberty and order, and the bringing nearer by gigantic strides of the day when peace shall come to the whole earth." An Opportunity for Expansion Cuba, only ninety miles off the coast of the United States, presented American espansionists with an opportune chance to end their country's isolation and assert its power in world affairs. Spanish control over the island had weakened considerably since the late 1860s, when Cuban rebels first began to agitate for their independence. In the 1870s a prolonged uprising took place against the Spanish; and in 1895, a revolution began.

She took part in the destruction of the Spanish Fleet at the Battle of Santiago off Cuba on July 3, one of the two major naval engagements of the war. The USS INDIANA was one of the new fleet of battleships promoted a special board created in 1890 to recommend the manner in which to update the U.S. Navy so that it could defend adequately against the imperial world powers, as well as by the nations of South America (several of which had larger vessels than any in the U.S. Navy). The board realized the need for battleships with a long-range cruising capability. However, many members of the United States Congress were in favor of Isolationism, and opposed long-range battleships as they were obviously not intended strictly for coastal defense. To assuage fears of the Isolationists, the new class of vessels were called "Sea-going coast-line Battleships". The name, an all-covering oxymoron, was thought to be particularly brilliant and useful by Theodore Roosevelt after he became Assistant Secretary of the U.S. Navy. In the Revolutionary War and the War of 1812, the American States achieved, and then defended, their independence. In the Mexican-American War, the United States established its Continental dominance. In World War II, the US halted the most violent and destructive ideology to date, and established itself as the clear leader of the free world. In the Spanish-American War, the US laid the foundation for its leadership role in WW II (and WW I), the Cold War, and beyond. It was the SAW which provided international recognition of the US as a great power. At the same time, the SAW had two major impacts within the US. One the one hand, it made Americans aware of a greater world around them, and of the need for the US to play its part. By defeating Spain hands down (after much speculation concerning the size and might of Spain's armed forces) Americans also gained self confidence in the ability of the US to project power abroad. On the other hand, the SAW was promoted largely as a "crusade" against an evil regime – much like both World Wars, like Korea and Vietnam, and like the ongoing war against terrorism. Unlike previous American conflicts which were largely fought for practical reasons – defeating Indian raiders, opening up new territories for settlements, acquiring resource-rich lands – the SAW was ostensibly fought for two reasons: liberating the oppressed people of Cuba from what was perceived as the Spanish whip (Spain played into US hands by establishing concentration camps and a brutally repressive regime to deal with local independence movements) and exacting revenge for the bombing of the Maine (attributed at the time to Spanish agents).
