The care was not comprehensive and often consisted of a single visit, typically lacking continuity of care, cultural sensitivity and a vision of coordinated prevention activities. These services obtained from multiple providers were generally poorly coordinated and lacked a systematic approach to the development of prevention activities as well as a lack of incentive to control medical costs.

The eighteenth century, in particular, represents the West as we think about it before the arrival of the white man.
