Research Tobacco research at Lombardi spans behavioral, genetic, epidemiology, basic science and policy, with a specific focus on harm reduction. We adopt a transdisciplinary approach to the biobehavioral model for smoking behavior and harm. An important area of Lombardi's tobacco research is harm reduction as a method of reducing risk. What is harm reduction? Following are brief descriptions of the ongoing tobacco research studies at Lombardi. ADHD and Smoking Behavioral Research in the National Lung Screening Trial The Lombardi Comprehensive Cancer Center is a national study site for both the Prostate, Lung, Colon, and Ovarian Cancer Screening Trial (PLCO) and the National Lung Screening Trial (NLST). Both trials address major research questions about screening for lung cancer in current and former smokers. The principle investigator for the PLCO and NLST trials is Edward Gelmann, MD. Matthew Freedman, MD, MBA, is lead radiology investigator for PLCO and principal co-investigator of NLST at Georgetown University. The PLCO Trial is closed to accrual, but has randomized 155,000 men and women to screening versus observation. Included in the screening arm is an annual chest X-ray for lung cancer detection. NLST addresses the risks and efficacy of a new approach to lung cancer screening, use of spiral computerized tomography (CT). Participants in this trial were randomized to annual chest X-ray or spiral CT examinations for 3 years. This trial is also closed to accrual and screening of 50,000 participants is ongoing at Lombardi and other centers a cross the country.

Its aim is to make children, teachers, parents and communities more aware of the opportunities that exist in schools for improving health. The Government signaled its intention to help all schools become healthy schools in the White Paper, 'Excellence in Schools', produced by the Department for Education and Skills (DFES) in 1997. The Healthy Schools Programme was announced in the Department of Health's (DH) Green Paper 'Our Healthier Nation', and launched on 14th May 1998 by Ministers Tessa Jowell (Public Health) and Estelle Morris (School Standards). The National Healthy School Standard (formerly known as the National Healthy Schools Scheme) was proposed in the Green Paper 'Our Healthier Nation' in 1998. In this paper, the government identified the school as a setting to improve the health of children. In the government's view, a healthy school is in a key position to improve children's health and educational achievement, with the support of other agencies.

Extractions: Public Policy of ASAM Nicotine is the psychoactive drug in tobacco. Regular use of tobacco products leads to addiction in a high proportion of users. Nicotine dependence is the most common form of chemical dependence in the United States. This addiction is especially prevalent among those who suffer from alcoholism and from other drug dependencies. Nicotine dependence most often begins as a pediatric disease. In 1994, four million young people, aged 12-17 years, were current users of cigarettes. Three thousand youth become regular users each day, one-third of whom will eventually die from a cigarette- caused disease. The nicotine addiction epidemic is fueled in part by the low price of tobacco products, their ready availability to the underage (despite laws to the contrary), and the enormous marketing campaigns for these products (campaigns that are often very seductive and attractive to the young). In 1993, the cigarette industry spent $6.03 billion, or 25 cents per pack, on marketing, which makes this industry among the largest, if not the largest, provider of health-related information in the country. Cigarettes cause an enormous burden of illness, disability and death. In 1989, the cigarette caused more than 400,000 premature deaths in the United States and more than 3 million worldwide.

The number of deaths (more than 400,000 annually) caused by tobacco use is greater than the combined number of deaths due to AIDS, alcohol, automobile accidents, murders, suicides, drugs and fires. Nicotine, a key ingredient in tobacco products, is an addictive drug. Tobacco use by and around children and adolescents is of particular concern due to increased risk for addiction and passive exposure. Smoking is a known cause of cancer, heart disease, stroke and chronic obstructive pulmonary disease. [Bibliography] Whereas the prevalence of tobacco use has declined in the general Canadian population in the last decade, the trends among the Native population are alarming . Apart from having much higher consumption rates for both smoke and smokeless tobacco, Native people also tend to start smoking earlier than non-Natives. In addition, there seems to be a certain lack of concern or awareness concerning the serious tobacco-related risks to the present and future health of many Native communities. Given the current levels of tobacco use among Native people, any effective reduction strategy will have to include at least three components, namely Protection, Prevention and Cessation. Since Environmental Tobacco Smoke (E.T.S.) is a serious health hazard to which non-smokers as well as smokers are continuously exposed, the first step is to ensure that the community has as many

Extractions: Summary INTRODUCTION Tobacco use is the single most preventable cause of death in the United States (1). Illnesses caused by tobacco use increase demands on the U.S. health-care system; lost productivity amounts to billions of dollars annually (2-3). Because four out of every five persons who use tobacco begin before they reach adulthood (1), tobacco-prevention activities should focus on school-age children and adolescents. Evidence suggests that school health programs can be an effective means of preventing tobacco use among youth (4-7). The guidelines in this report have been developed to help school personnel plan, implement, and assess educational programs and school policies to prevent tobacco use and the unnecessary addiction, disease, and death tobacco use causes. Although these guidelines address school programs for kindergarten through 12th grade, persons working with youth in other settings also may find the guidelines useful. The guidelines are based on a synthesis of results of research, theory, and current practice in tobacco-use prevention. To develop these guidelines, CDC staff convened meetings of experts from the fields of tobacco-use prevention and education, reviewed published research, and considered the conclusions of the National Cancer Institute Expert Advisory Panel on School-Based Smoking Prevention Programs (4) and the findings of the 1994 Surgeon General's Report, Preventing Tobacco Use Among Young People (8).

Life expectancy, which was 72.5 for males and 79.3 for females in the year 1995, was shortened by diseases such as cancer, pulmonary, heart and cerebrovascular disease. These diseases can be directly associated with cigarette smoking. An estimated 4.5 million children and adolescents are now smoking. Nadelmann feels, According to the Chronic Disease Preservation (CDC) the average age at which children try cigarettes is 11.6 years with children as young as eight years of age planning to or already having experimented with smoking. The National Household Surveys on Drug Abuse estimates that six thousand or more young people try their first cigarette daily, and three thousand become daily smokers each day. A student who is not a smoker by age 18 is unlikely to start as an adult. It seems that smokers in their teen years regret their decision to smoke and 66% say they want to quit, but it is too hard.

Ac Michael Smith, MD. Lincoln Detox Program 349 E. 140th St, Bronx N.Y. The Lincoln Detox Program has been using acupuncture to treat drug abuse problems for three years. We have helped more than one thousand patients. Acupuncture allows people addicted to heroin and methadone to detoxify more comfortably, more quickly, and with far less narcotic intake than with the standard method of methadone detoxification. Severe prolonged withdrawal symptoms of methadone addiction -such as insomnia, bone pain, depression are untreatable by any other means. Acupuncture also reduces craving for drugs by strengthening normal physiological processes and helping the person cope with anxiety in a relaxed manner. Acupuncture is considered a method of natural healing, because it works by stimulating the flow of natural health-giving energies in the body. In contrast, medication such as penicillin or methadone act by altering certain biological cells. Often the lightest touch of a finger is just as effective or more effective as an acupuncture treatment than relatively strong electrical stimulation. We cannot explain or understand acupuncture in the same way that we explain Western medical treatments. 10mg. of morphine has a predictable effect on the perception of pain all over the body. Stimulation of the Ho Ku point on the hand does not have the same kind of predictable effect. Differences in the balance of body energy flow will have a great effect on the results of Ho Ku stimulation. Acupuncture therapists can never rely on standardized treatment plans but always must discover the day-to-day changes in the patients energy flow.
