Extractions: Reyna Shah Community Health Scholars Final Report Supervisors: Toni Sell and Gil Walters There were three major goals of this project, all of which emphasized the importance of medical awareness and education within the vulnerable populations of rural Northeast Florida. The counties that were targeted were Putnam, Alachua, Clay, and St. Johns. The respective cities were Palatka, Crescent City, Hawthorne, Keystone Heights, and Hastings. The first step in accomplishing goal one was to locate the senior centers in surrounding counties. After contacting senior center coordinators in Palatka, Hawthorne, Hastings, Keystone Heights and Bunnell, a consensus was reached on the two most important yet poorly understood diseases. These two diseases were Hypertension and Diabetes Mellitus. These diseases seemed of particular interest to the senior centers of Northeast Florida because of their astounding prevalence and ensuing complications due to lack of disease management. This type of educational outreach was presented in both clinical and seminar settings. Several resources were used in an effort to disseminate comprehensive, concise yet useful information about Hypertension and Diabetes Mellitus. The focus of the seminars was pathology and preventive methods such as proper nutrition, regular monitoring, diet and medication management. When I addressed the two diseases at the senior centers, the first topic was basic anatomy of the heart and pancreas.

(5) "Approved form" means (A) Supreme Court approved forms or (B) forms that have been approved in writing by the chief judge of a circuit and that are not inconsistent with the Supreme Court approved forms, copies of which are to be sent to the Chief Justice, the chair of the Family Law Rules Committee of The Florida Bar, the chair of the Family Law Section of The Florida Bar, and the chair of the Family Court Steering Committee. Forms approved by a chief judge may be used unless specifically rejected by the Supreme Court.
