PHILADELPHIA, July 9, 1996 During this summer's Olympic games, no matter who the big winners are, it's almost certain that one of the continuing news stories will revolve around use of banned substances by athletes. But what may be a much more important story has gone under-reported: the deliberate use of performance-enhancing drugs by athletes at all levels of competition in the United State. So suggest the authors of a new book, "Warning: Drugs in Sports A Handbook for Every Athlete and Parent" (published by Medical Surveillance, Inc.). "Performance-enhancing drugs have penetrated college, high school and junior high/middle school athletics," explained co-author Dr. Andrew Ferko, associate professor of Pharmacology, Allegheny University of the Health Sciences, Philadelphia. Though many sports programs have instituted drug screenings, they are often too narrow to catch most drugs or can't keep up with innovations in performance-enhancing drugs. For example, the drug Erythropoietin, used to increase the blood's ability to carry oxygen, will not be screened at the Olympics.

By Jessie A. (Grade 7) Twenty per cent of athletes in Western countries admit to using drugs. That's one out of every five athletes, and that's only those who admit to it. When you're watching sports on television, you have no idea if what you are seeing is the athlete's true abilities, or steroids in effect. A program of random drug tests, education, treatment, and discipline would cost an estimated one million dollars annually. But one million dollars may be a small price to pay for fair play and good health. That's why random drug tests would be extremely effective if taken by the athletes, not only throughout the season, but throughout the rest of the year as well. Drugs shouldn't be tolerated in any team, in any sport. Steroids and other performance-enhancing drugs have many health risks. Risks with steroids include heart disease, liver tumors, and edema (abnormal fluid accumulation in body tissues). The athletes should ask themselves, "Is this really worth my life?" Glen Sather, Edmonton Oilers general manager and coach, says, "In my mind I'm absolutely convinced there is no drug abuse problem on my team." Sather could sadly be mistaken.

LINDEN, NJ. -The New York Times recently ran a story stating the International Olympic Committee said drug testing in many sports has been very lax and the same appears to be true for the upcoming Olympic Games. The article also stated that record breaking performances in any sport involving strength and speed would be suspect. Steve Holman, one of the United States' premier milers, was quoted as saying that the report's conclusions on the efficiency of drug testing reiterated what he and others had been saying for years. Before the 1980s drugs were associated only with strength and gaining weight and were not believed to offer any benefit to runners. Thus shot putters, hammer throwers, discus, and javelin throwers were the ones who were observed with an occasional raised eyebrow and skeptical eye. Now, especially since the Ben Johnson debacle in 1988 and the years later suspension and reinstatement of 400 meter runner Butch Reynolds, sprinter Merlene Ottey's suspension last year and the similar plight of other athletes, track and field has become a sport where mental asterisks accompany many performances. How did all this start?

The use of drugs by amateur and professional athletes has received much publicity in recent years. Many athletes believe that drugs are essential for optimum performance. The issue of whether drugs can enhance physical performance remains controversial and unresolved. However, the physiological effects that drugs have on the body can be documented. This section is devoted to examining the most-common drugs used by athletes. Questions of legality or ethics are best answered by the prevailing view in sports medicine: The use of drugs is generally considered unethicalin some cases illegal and such use is usually forbidden by organizations that govern competitive athletics. ANABOLIC STEROID HORMONE
Some athletes take synthetic male hormones (anabolic steroids) in the hope of increasing strength or muscle mass. The most common synthetic male hormones taken by athletes include testosterone, methandrostenolone and nandrolone. EFFECTS IN WOMEN Muscle mass increases when the hormone is taken for a sufficiently long period of time. However, side effects and adverse reactions include the following:

The report in the popular media that some track-and-field athletes have been using a so-called "designer steroid" created to thwart drug testing regimens is the latest installment in the ongoing battle against doping, or the use of performance-enhancing technologies, in sports. Are sports organizations responding appropriately to these challenges, and what is the proper role of physicians in this controversy?
Backround
According to the World Anti-Doping Agency, the term "doping" probably comes from the Afrikaans word "dop," a concoction made from grape leaves that Zulu warriors drank before going into battle. In sports, the term was first used to describe the illegal drugging of race horses at the beginning of the 20th century.
Doping in sport now includes a range of practices, including "blood doping" (the practice of autologous or homologous hemoglobin transfusions) and the use of synthetic erythropoeitin (EPO) to increase the number of red blood cells; anabolic steroids and human growth hormone to grow skeletal muscle; stimulants to improve cognitive function and reduce fatigue; and nitrogen tents and "houses" to simulate the effects of sleeping at high altitude. The future holds the promise of more powerful and exotic interventions.

Jeffrey Brown discusses the widening investigation into steroids in sports with New York Times sportswriter Liz Robbins and Dr. Gary Wadler, a drug-policy adviser for the White House and the NBA. SPORTSCASTER: ( Cheers and applause ) Look how fast she stomped on the athlete out in front of her. JEFFREY BROWN: One of America's top track athletes admitted using performance-enhancing drugs yesterday, and was suspended from racing for two years. Sprinter Kelli White, who won the 100- and 200-meter races at the world track and field championships last summer, will now miss this summer's Olympic games in Athens. The 27-year-old had been seen as a gold medal contender. The suspension is the latest in a growing drug scandal in American sports, and it was highly unusual. White did not fail a traditional drug test. Instead, she admitted using steroids and a blood-boosting agent after being confronted by the U.S. Anti-Doping Agency with documents and other evidence. The USADA manages drug testing of athletes in Olympics-related sports. The evidence in the White case came from a federal investigation of a company known as BALCO, the bay area laboratory cooperative. Its owner and several others were charged this winter with conspiring to sell banned drugs to athletes.
