When Kocher began his surgical activities the transition from the septic to the antiseptic treatment of wounds had been completed and Kocher worked for the latter with all his energies because of its great importance. Then arose a series of works on the antiseptic treatment of wounds with weak chlorine solutions, the Listerian treatment of ovariotomies (1875), the preparation of antiseptic catgut and the simplest method of obtaining healing of wounds without drainage tubes, on conditions governing healing by first intention, and on Lister's wound dressings. The surgical clinic in Berne was for a long time the centre of attraction for medical men who favoured the antiseptic treatment of wounds and wished to follow it. Later Kocher was one of the first to go over to pure asepsis about which he had the best opportunities to learn through his collaboration with Tavel, whose bacteriological studies on infective processes he sought to advance. From this work proceeded the second edition of Vorlesungen über chirurgische Infektionshrankheiten (Lectures on surgical infectious diseases ) by Kocher and Tavel, Basel, 1892, and Jena, 1900.
His Chirurgische Operationslehre (Theory on surgical operations) reached six editions and was translated into most modern languages. It described many operations, mostly in abdominal surgery and the surgery of joints. His book Erkrankungen der Schilddrüse (Diseases of the thyroid gland ) discussed the etiology, symptology and treatment of goitres. His new ideas on the physiology and pathology of the thyroid gland caused controversy.

    • Name: Emil Theodor Kocher Dates: 1841-1917 Nationality: German Place of Birth: Berne Last Post: Professor of Surgery at Berne Last ward round note: ...and I pulled onto the hard shoulder only to find the fan belt had gone. Claim to fame: NoblePrize for work on the Thyroid gland 1909 Orthopaedics: Kocher's method of reduction of a dislocated shoulder 1870 General Surgery: Kocher's incision, manoeuvre and forceps (see text below)
Undoubtedly, an excellent surgical genius. Studied in London, Paris, Berlin and Vienna. Professor of Surgery aged 31. Apart from inventing the occasional surgical tool, he spent the average day figuring out and recording incisions and approaches which culminated into an astonishing book. Kocher's right subcostal incision or a double Kocher 'rooftop incision' with their subsequent approaches, gives wide exposure to the upper abdominal viscera. Kocher's manoeuvre gives access to the posterior duodenal surface and the head of the pancreas. In addition the lower bile duct is exposed. This is achieved via an incision along the peritoneum at the right edge of the duodenum (2nd part). One then turns the duodenum medially, taking extreme caution not to damage the ureter, gonadal vessels and the IVC.

The Swiss surgeon Emil Theodor Kocher (1841-1917) received the 1909 Nobel Prize in physiology or medicine for his work on the thyroid gland. This innovative work eventually resulted in effective treatment of thyroid disorders not only by the use of surgery but also by the use of such drugs as thyroid extract and thyroxin. The Swiss surgeon Emil Theodor Kocher made innumerable contributions to techniques in many surgical procedures, including operations on the lungs, stomach, and gallbladder. Especially important was his pioneering of ovariotomy and the application of antiseptic surgery, introduced by Joseph Lister (1827-1912). The surgical success of Kocher depended greatly on the many improvements he made in appliances and instruments. However, despite his versatility and creativity in surgery, Kocher is best known for his work on the physiology, pathology, and surgery of the thyroid gland, for which he was awarded the Nobel Prize for physiology or medicine in 1909. Kocher established not only the function of the thyroid gland but also the cause of cretinism and myxedema.

Emil Theodor Kocher Theodor Kocher was a pioneering surgeon of the last half of the nineteenth century. His major discoveries were in the field of surgery of the thyroid gland. The contributions of Theodor Kocher had a great impact on thyroid surgery. Theodor Kocher was born on August 25, 1841, at Berne. When he was 31 years old He was appointed professor of Surgery and Head of the University Clinic in Berne. He held this position until retirement. He was a student of Billroth and Langenbeck. He had a particular interest in infections, especially in the prevention of surgical infections. Kocher's studies on one of his patients, a eleven year old girl, who was operated on in 1874, led to the understanding on post operative myxoedema. This girl had a successful removal of her thyroid, but after the surgery, she became very tired, showed no signs of initiative and became cretinoid. She remained small and had an ugly and idiotic appearance in contrast to her sister. He also pioneered post op substitution of thyroid hormone.

    Essence of surgery
A Brief Journey Through Surgical History "So the Lord God caused a deep sleep to fall upon the man, and while he slept he took one of his ribs, and closed up its place with flesh." (Revised Standard Version) For those who believe in the Old Testament, this is the first documented account of a surgical operation. The first hard evidence of early surgical procedures came with the finding of trephined skulls some 10,000 years old. The indications for trephining (or trepanning) are unclear, but most authorities hold that it was done to release evil spirits, and as a treatment for epilepsy and migraine Although there is little evidence that the Ancient Egyptian practice of mummification led to detailed knowledge of human anatomy, the Edwin Smith papyrus from the 17 th BC provides one of the earliest written record of surgical procedures.
