Description
Lithium is an antipsychotic, antimanic medication used for those with Affective Disorder, Bipolar; Bipolar Disorder Depression; Mania . (Lithium medications may also go by the brand names: Eskalith, Eskalith-Cr, Lithane, Lithium, Lithobid, Lithonate, Lithotabs.)
Pharmacology
Lithium is an element of the alkali-metal group. Preclinical studies have shown that lithium alters sodium transport in nerve and muscle cells and effects a shift toward intraneuronal metabolism of catecholamines, but the specific biochemical mechanism of lithium action in mania is unknown.
Indications and Usage
Lithium carbonate is indicated in the treatment of manic episodes of manic-depressive illness . Maintenance therapy prevents or diminishes the intensity of subsequent episodes in those manic-depressive patients with a history of mania.

Summary
CARBOLITH
Lithium Carbonate Use:

Treatment of manic episodes of manic-depressive illness. Initiate treatment at 600-900 mg/day in 3 divided doses; increase to 1,200-1,800 mg/day in 3 divided doses to achieve serum lithium concentrations between 0.8 and 1.2 mmol/L. Adjust dose according to clinical response and trough serum level (12 h) after steady state is reached (4-5 days). After the acute manic episode subsides, adjust dose to maintain a level of 0.6-1.0 mmol/L. The average dosage range at this stage is usually 450-1,200 mg/day. If a satisfactory antimanic response is not obtained in 14 days, consider discontinuing lithium. Once patients are stabilized on a maintenance dose with a multiple dosing schedule and once stable therapeutic blood levels are reached, consider a once-daily dosage regimen. The total daily dose, when given as a single dose, may be about 5-30% lower than when given in divided doses over the day. Geriatrics: Starting dose should not exceed 300 mg/day accompanied by frequent serum level monitoring (concentrations should not exceed 1 mmol/L; usual range: 600-1,200 mg/day).

WARNING
Abnormal elevation of serum potassium levels (greater than or equal to 5.5 mEq/liter) can occur with all potassium-sparing diuretic combinations, including hydrochlorothiazide; triamterene. Hyperkalemia is more likely to occur in patients with renal impairment and diabetes (even without evidence of renal impairment), and in the elderly or severely ill. Since uncorrected hyperkalemia may be fatal, serum potassium levels must be monitored at frequent intervals especially in patients first receiving hydrochlorothiazide; triamterene, when dosages are changed or with any illness that may influence renal function.

The Department of Pharmacology at University College London was the first Pharmacology Department to be established in England in 1905. Mainly housed in a Grade II listed building but with several areas completely refurbished in 2005, it provides excellent facilities for teaching and research in Pharmacology. It is the only Department in the UK to have been rated in the top category in all five UGC/HEFCE research assessment exercises to date, and was the top-ranked (5 star A) Department in the last two exercises in 1996 and 2001. The rest of the Department is housed in the second floor of the Medawar Building and in the top two floors of the new Andrew Huxley building. Our Laboratory for Molecular Pharmacology is situated in the Medical Research Council (MRC) building adjacent to the main department.

    Hydrochlorothiazide is a diuretic and antihypertensive. It is the 3,4-dihydro derivative of chlorothiazide. Its chemical name is 6-chloro-3,4-dihydro-2 H -1,2,4- benzothiadiazine-7-sulfonamide 1, 1-dioxide. Its empirical formula is C H ClN O S and its structural formula is: It is a white, or practically white, crystalline powder with a molecular weight of 297.72, which is slightly soluble in water, but freely soluble in sodium hydroxide solution. Hydrochlorothiazide is supplied as 25 mg, 50 mg and 100 mg tablets for oral use.
