HYG-2076-96
William F. Lyon It is estimated that between one and two million people in the United States are severely allergic to stinging insect venom. Each year 90 to 100 deaths from sting reactions are reported, but many more deaths may be occurring, mistakenly diagnosed as heart attacks, sunstrokes or attributed to other causes. More people die each year from the effects of insect venom than from spider or snake bites. Extreme human sensitivity to stings resulting in serious or fatal reactions is confirmed almost entirely to cases involving bees, wasps, hornets, bumble bees and ants (Order Hymenoptera). Unlike most other allergies, insect allergy can cause a life-threatening disruption to breathing and circulatory systems called anaphylactic shock. For one person in 100, the sting of an insect can be fatal.
Allergic Reaction Symptoms
Most people stung will experience a "local" reaction with redness, pain, swelling and some itching only at the sting site. If the reaction progresses quickly to sites other than the sting site or is followed by difficult breathing or choking at the throat, the person is experiencing a "systemic" allergic reaction (anaphylaxis) requiring emergency medical treatment. Remember that if you are stung on the hand and your face begins to swell or hives break out all over your body, this is a serious condition requiring emergency room attention.

Chinese scientists have inserted scorpion and moth genes into oilseed rape (canola) plants to make them poisonous to insects feeding on them. The researchers say that using two foreign genes at the same time means insect pests will be less likely to develop resistance to the genetically modified (GM) plants. The findings were published online in Plant Cell Report on 19 July. The researchers tested the GM plants by exposing them to caterpillars of the diamondback moth (Plutella maculipenis) . All of the plants showed some defense against the caterpillars, although the extent of this varied from plant to plant. The caterpillars, which are major crop pests in parts of Africa, Asia and Latin America, have developed resistance to several available pesticides. There are also reports they are becoming resistant to 'Bt' plants, which are genetically modified to produce a toxin that kills insects but is harmless to birds and mammals. The Chinese team, led by Jingxue Wang of the Agri-Biotechnology Research Centre of Shanxi Province, inserted two genes at once to overcome this problem.

(Backflap in suitcase [LARGER IMAGE] : FBI. Detail of backflap [LARGER IMAGE] : University of Pennsylvania Museum.) The strange journey of a priceless Moche artifactfrom Peru to Panama, to New York, and finally to Philadelphia via the New Jersey Turnpikeended when the FBI, which recovered it in a sting operation, presented it to Peruvian ambassador Ricardo Luna at the University of Pennsylvania Museum of Archaeology and Ethnology on July 15. The object, known as a backflap, was part of the costume worn by elite members of Moche society, which flourished on the coastal plain of northern Peru from 200 B.C. to A.D. 700. Two of the three men involved in smuggling the backflap are in jail. The third, a former consul general of Panama, has been indicted. Made of gold, copper, and silver, the backflap weighs about 2.5 pounds and is 25.6 inches long and 19.6 inches wide. It consists of flat blade-shaped central piece surmounted by rattles made of matching front and back pieces. Known from tombs of Moche warrior-priests and depictions on vases, backflaps were suspended from a belt around the waist and covered the wearer's backside. Warrior-priests wore them as armor in combat and as symbols of power during rituals including the sacrifice, perhaps to insure rainfall and agricultural fertility, of captured enemy warriors. The semicircular rattle is decorated with a central figure known to archaeologists as "the Decapitator" because he is shown holding a sacrificial knife, or

    INTRODUCTION
Toxicologically, there is far more diversity in spider venoms than in the venoms of Hymenoptera and scorpions, so that the correct identification of spider bites has special significance. Spiders are rarely correctly identified by bite victims or their physicians and are readily transported by human commerce out of their native range. There are stellar reviews of the toxicology of arthropod venoms and poisons , as well as an excellent quick guide to bites and stings of arthropods
    BITE VS. STING
    THE STINGING HYMENOPTERA
Evolution of the sting The most common stinging animals are in the insect order Hymenoptera (meaning "veil wings"), which includes the bees, wasps and ants. The most primitive Hymenoptera possess ovipositors to insert eggs into plant tissue. In some parasitic groups this structure and glands associated with it have been modified to inject venom to paralyze other insects which are used as food by the developing larvae. (These parasitic wasps comprise the largest number of species in the Hymenoptera, and are extremely beneficial to agriculture as biological control agents of agricultural pest insects.) The stings of these parasitic wasps are not usually very painful to humans, and their venom has low mammalian toxicity

Department of Emergency Medicine, King Fahad National Guard Hospital, Riyadh 11426, Saudi Arabia. Accepted for publication 8 August 1994. Ann Saudi Med 1995;15(2):148- 155. Scorpions are widely distributed in Saudi Arabia. Fourteen species or subspecies, belonging to two families, the Buthidae and the Scorpionidae , have been identified.1 Nine of these have been found in the Riyadh area (Table 1). The two most venomous species appear to be Leiurus quinquestriatus and Androctonus crassicauda of the family Buthidae . The physician in the Emergency Department may be unsure of the optimal management after envenomation and there is often a great deal of anxiety on the part of the patient as to the course the illness may take. In certain areas of the Kingdom, scorpion stings result in significant morbidity and even mortality.2,3 Toxicity in Saudi Arabia, however, is variable. Neale reported 205 cases of "scorpion sting syndrome" presenting to the King Fahad Hospital in Riyadh over a five-year period beginning in 1983.4 Significant morbidity was found to be rare and there was no mortality. It was the purpose of this retrospective review to further assess the risk and morbidity of envenomation by scorpion stings in the Riyadh area, to determine whether antivenin therapy would be of benefit in this region of the country. The study is an extension of the review by Neale and encompasses the 10-year period from 1983 to 1992. It is hoped that the data presented will help guide practitioners in the management of patients presenting with "scorpion sting syndrome".
