AIMS AND SCOPE
The great advances in immunology in recent years make this field one of the most rapidly growing in biological sciences. This remarkable growth is stimulated by the influx of investigators from other disciplines such as biochemistry, genetics, molecular biology, and by an increased number of investigators who came to immunology through the more traditional routes of microbiology and various medical disciplines. As a consequence, immunology has become a vast and rich field encompassing outlooks that range from the highly clinical to the highly molecular. Although such perspectives may appear diverse, they are, in fact, highly interdependent. Critical Reviews TM in Immunology presents a balanced overview of contemporary immunology and melds together molecular immunology and immunobiology.

    Evolution of the Immune System
    The immune system consists of factors that provide innate and acquired immunity, and has evolved to become more specific, complex, efficient, and regulated. One of the principal functions of the human immune system is to defend against infecting and other foreign agents by distinguishing self from non-self (foreign antigens) and to marshal other protective responses from leukocytes. The immune system, if dysregulated, can react to self antigens to cause autoimmune diseases or fail to defend against infections.
    Organization/Components/Functions
    The immune system is organized into discrete compartments to provide the milieu for the development and maintenance of effective immunity. Those two overlapping compartments: the lymphoid and reticuloendothelial systems (RES) house the principal immunologic cells, the leukocytes. Leukocytes derived from pluripotent stem cells in the bone marrow during postnatal life include neutrophils, eosinophils, basophils, monocytes and macrophages, natural killer (NK) cells, and T and B lymphocytes. Hematopoietic and lymphoid precursor cells are derived from pluripotent stem cells. Cells that are specifically committed to each type of leukocyte (colony-forming units) are consequently produced with the assistance of special stimulating factors (e.g. cytokines). Cells of the immune system intercommunicate by ligand-receptor interactions between cells and/or via secreted molecules called cytokines. Cytokines produced by lymphocytes are termed lymphokines (i.e., interleukins and interferon-gamma) and those produced by monocytes and macrophages are termed monokines.

    Clinical and training programs
    Clinical experience is drawn from patients on general medical, surgical, pediatric and ob/gyn services, including specialized areas such as hemodialysis, oncology, cardiac surgery, neonatology, ophthalmology, renal transplantation, orthopedics andurology. Diagnostic microbiology includes antibiotic assays, anaerobic bacteriology, virology, mycology, mycobacteriology, serology, and parasitology in addition to routine bacteriology. Infection control activities include device-related infections, antibiotic surveillance and laboratory studies.
    The infectious diseases fellowship program consists of two pathways for training. One track, designed for two years, has a primary emphasis on training in clinical infectious diseases, diagnostic microbiology, and infection control. This track prepares the fellow for a career in the clinical practice of infectious diseases. The second pathway, which is for at least three years, offers combined clinical and research training. This track prepares the fellow for an academic career and consists of one year of clinical training and the remainder of time in research which is supported by a National Institutes of Health training grant. The program also has a rotating weekly interhospital clinical conference, a monthly lecture series, a biweekly journal club, and weekly clinical conferences on both general infectious disease management and HIV disease which residents are encouraged to attend.

    corrections to Dr Richard Hunt PART ONE: IMMUNOLOGY Immunology is the study of our protection from foreign macromolecules or invading organisms and our responses to them. These invaders include viruses, bacteria, protozoa or even larger parasites. In addition, we develop immune responses against our own proteins (and other molecules) in autoimmunity and against our own aberrant cells in tumor immunity. Our first line of defense against foreign organisms are barrier tissues such as the skin that stop the entry of organism into our bodies. If, however, these barrier layers are penetrated, the body contains cells that respond rapidly to the presence of the invader. These cells include macrophages and neutrophils that engulf foreign organisms and kill them without the need for antibodies. Immediate challenge also comes from soluble molecules that deprive the invading organism of essential nutrients (such as iron) and from certain molecules that are found on the surfaces of epithelia, in secretions (such as tears and saliva) and in the blood stream. This form of immunity is the innate or non-specific immune system that is continually ready to respond to invasion.
